چکیده

Question: In patients with clinical stage IIA through IIC cutaneous melanoma thicker than 2 mm, is survival different for a 2-cm local excision margin compared with a 4-cm excision margin? Design: Randomized controlled trial. The trial was not masked at any stage. Setting: Multicenter trial in 53 hospitals in 9 European centers in Sweden, Denmark, Estonia, and Norway. Patients: Patients 75 years or younger with primary cutaneous melanoma thicker than 2 mm and with clinically localized disease (clinical stage IIA-C), on the trunk or upper or lower extremities. Intervention: Wide 2-cm local excision margin compared with a 4-cm excision margin. Main Outcome Measures: The primary outcome was overall survival. Secondary outcomes were recurrence-free survival and the number of local recurrences. Adverse events were not systematically recorded. Results: A total of 936 patients were enrolled from January 22, 1992, to May 19, 2004; 465 were randomly allocated to treatment with a 2-cm resection margin, and 471 received treatment with a 4-cm margin. After a median follow-up of 6.7 years, 181 patients in the 2-cm margin group and 177 in the 4-cm group had died (hazard ratio, 1.05; 95% CI, 0.85-1.29) (P=.64). Five-year overall survival was 65% (95% CI, 60%-69%) in the 2-cm group and 65% (95% CI, 60%-70%) in the 4-cm group (P=.69). Recurrencefree survival did not differ between the 2 groups (P=.82). The proportion of relapse-free patients at 5 years was 56% (95% CI, 51%-61%) in both treatment groups. Conclusions: Findings suggest that a 2-cm resection margin is sufficient and safe for patients with cutaneous melanoma thicker than 2 mm.

2-cm versus 4-cm surgical excision margins for primary cutaneous melanoma thicker than 2 mm: a randomised, multicentre trial.

BACKGROUND Optimum surgical resection margins for patients with clinical stage IIA-C cutaneous melanoma thicker than 2 mm are controversial. The aim of the study was to test whether survival was different for a wide local excision margin of 2 cm compared with a 4-cm excision margin. METHODS We undertook a randomised controlled trial in nine European centres. Patients with cutaneous melanoma t...

Adapted ECHO-7 virus Rigvir immunotherapy (oncolytic virotherapy) prolongs survival in melanoma patients after surgical excision of the tumour in a retrospective study

An oncolytic, nonpathogenic ECHO-7 virus adapted for melanoma that has not been genetically modified (Rigvir) is approved and registered for virotherapy, an active and specific immunotherapy, in Latvia since 2004. The present retrospective study was carried out to determine the effectiveness of Rigvir in substage IB, IIA, IIB and IIC melanoma patients on time to progression and overall survival...

Evaluation of outpatient discharge in patients with cutaneous melanoma.

OBJECTIVE to evaluate discharge in a group of patients with cutaneous melanoma according to recently established criteria. METHODS we conducted an observational, cross-sectional study with 32 patients at the Hospital Universitário Clementino Fraga Filho (HUCFF) / Universidade Federal do Rio de Janeiro (UFRJ), between 1995 and 2013, in the following stages: IA (17 cases, 53.12%), IB (4 cases, ...

Optimal excision margins for primary cutaneous melanoma: a systematic review and meta-analysis.

OBJECTIVE To determine in patients with localized primary melanoma of the trunk or extremities the optimal excision margin that achieves the highest disease-free survival and overall survival and the lowest local recurrence rate. DATA SOURCES Trials comparing 2 different excision margins were identified by searching MEDLINE from 1966 to May 2002 using the term "melanoma," subheading "surgery,...

Surgical excision margins for primary cutaneous melanoma.

BACKGROUND Cutaneous melanoma accounts for 75% of skin cancer deaths. Standard treatment is surgical excision with a safety margin some distance from the borders of the primary tumour. The purpose of the safety margin is to remove both the complete primary tumour and any melanoma cells that might have spread into the surrounding skin.Excision margins are important because there could be trade-o...
